Red City Fitness Liability Waiver
Red City Fitness Participant Release of Liability and Assumption of Risk Management
I hereby waive and release, hold harmless and forever discharge Red City Fitness all claims, demands, cause of action, damages and liabilities of every kind and nature, arising from or in any way related to my participation in any training plans or macronutrient and nutrition guides given by Red City Fitness, provided that this waiver of liability does not apply to any gross negligence or intentional or willful misconduct. The training plans include but is not limited to in person and online personal trainings, partner trainings, challenges, and classes.
I understand that the activities in which I will participate are inherently dangerous and can cause serious injury including bodily injury, damage to personal property and/or death. I waive all claims of injury to my body and property and death that I may have against the aforementioned company pertaining to such activity, including claims in tort, contract, equity, or otherwise.
I acknowledge, agree and represent that I understand the nature of the training and its activities and that I am qualified, in good health, and in proper physical condition to participate in related activities. That I am physically sound and have medical approval to proceed with a normal routine of exercise. That I am in good health and have no physical conditions that would be aggravated by my involvement in cardiovascular exercise, weight lifting, weight training, bodybuilding, nor do I have any physical limitations that would preclude said involvement. I further agree and warrant that if at any time I have questions or feel unsafe, I will immediately discontinue further participation in the activities.
Description of potential benefits:
I understand that a program of personal training or group training classes is for the heart, muscles and joints, has many benefits associated with it. These may include a decrease in body fat, improvement in blood fats and blood pressure, improvement in physiological function, stress, and decrease in heart disease.
I agree I shall undertake all exercises at my own risk. I acknowledge that Red City Fitness trainer is not a certified nutritionist or dietician and that the macronutrient/ nutrition guides given are a guide that I will follow at my own risk, should I choose to do so. I agree to be solely responsible for safety and well-being of myself.
My signature is proof of my intention to execute a complete and unconditional waiver and release of all liability to the fullest extent of the law. I am at least 18 years old and am mentally competent to enter into this agreement.
